چکیده

Group A Streptococcus (GAS) is known to cause infections such as pharyngitis, pneumonia, impetigo, erysipelas, necrotizing fasciitis, and septicemia, and can be complicated by rheumatic fever, glomerulonephritis, and erythema nodosum [1]. In 1987, Cone et al described two patients with severe GAS infections who had clinical features similar to staphylococcal toxic shock syndrome (TSS). This syndrome was designated streptococcal TSS [1]. Due to the availability of effective antimicrobial agents, improvements in sanitation, or a probable decrease in the virulence of the microorganism, the frequency and severity of these infections has declined [2]. However, in the past few years, there appears to have been a change in the spectrum of disease caused by GAS, including more frequent reports of streptococcal TSS. The reemergence of serious infection is probably related to the renewed prevalence of more virulent strains and more susceptible host factors [3]. Here, we report a case of streptococcal TSS with the initial presentation of peritonitis and shock. A high degree of suspicion is important to make this diagnosis and to achieve good therapeutic results.

Group B Streptococcal Toxic Shock Syndrome and covR/S Mutations Revisited

Gene mutations in the virulence regulator CovR/S of group A Streptococcus play a substantial role in the pathogenesis of streptococcal toxic shock syndrome. We screened 25 group B Streptococcus (GBS) isolates obtained from patients with streptococcal toxic shock syndrome and found only 1 GBS clone harboring this kind of mutation.

Streptococcal toxic-shock syndrome: spectrum of disease, pathogenesis, and new concepts in treatment.

Since the 1980s there has been a marked increase in the recognition and reporting of highly invasive group A streptococcal infections with or without necrotizing fasciitis associated with shock and organ failure. Such dramatic cases have been defined as streptococcal toxic-shock syndrome. Strains of group A streptococci isolated from patients with invasive disease have been predominantly M type...

Toxic shock syndrome due to group A streptococcal pharyngitis and bacteremia in adults.

Bacteremia and/or toxic shock syndrome are rare complications of streptococcal pharyngitis in adults. This report describes a previously healthy young man with streptococcal toxic shock syndrome who presented with fatigue, high fever, and suspected extensive streptococcal tonsillopharyngitis. Treatment consisted of high-dose antibiotics followed by therapy for consumptive coagulopathy, acute re...

Toxic Shock Syndrome within 24 H of an Office Hysteroscopy

Office hysteroscopy is now a common procedure performed to look at the endometrial cavity and is relatively free of serious complications. A 68-year-old lady, previously fit and well, presented with abdominal pain, rigors, sweats, and vomiting within 24 h of an outpatient hysteroscopy for postmenopausal bleeding. She was diagnosed with streptococcal toxic shock syndrome (STSS) due to Group A be...

Toxic shock syndrome-associated staphylococcal and streptococcal pyrogenic toxins are potent inducers of tumor necrosis factor production.

Toxic shock syndrome-associated staphylococcal and streptococcal exotoxins were tested for an ability to induce the production of tumor necrosis factor (TNF). Staphylococcal enterotoxins B and C1, along with streptococcal pyrogenic exotoxin A, all induced TNF production in a dose-dependent manner, with production peaking on the average at 3 days but continuing over the 6 days tested. This time ...
